go to "submit deviation" at the top of the page on the right hand side. Then it will take you to a different page that asks you to describe your submition, w/ a title, type (there will be a drop down menue ) and then you click add file. U have to put something in comments box and in the key words box. There can be more than one key word and after that all you do is hit submit!
